Job description
Monday through Friday. 7:30am to 3:30pm with benefits. RN would be responsible for
completing in-home assessments and overseeing the service of clients by in-home caregivers. Very low stress, no skilled care, no weekends, all holidays off, no evenings or on call times, very rewarding.
Responsibilities and Duties
Include but are not limited to...
- Use policy and procedure that regulates programs to complete assessments of Client needs via phone and in-person
- Create individualized and comprehensive care management and treatment plans
- Review caregiver worksheets
- Position is based in Saint Albans with some local travel required
Qualifications for Registered Nurse Case Manager
- Associate's or bachelor's degree in nursing
- Active nursing license in the state of West Virginia
- Demonstrated ability to assess and respond to client and family needs in a timely manner
