Position: RN Case Manager

Job Summary: Demonstrate good leadership, communication, and writing skills. Enforce the policies and procedures of the agency. Follow state and federal laws as applicable and mandated.

Qualifications/Educational Requirements:

1. Registered Nurse licensed to practice in the State of NC.

2. Must be a “health care practitioner" which means an individual who is licensed, certified, or registered to engage in the practice of medicine, nursing, dentistry, pharmacy, or any related occupation involving the direct provision of health care to patients.

3. At least two years of supervisory or administrative experience in home health or related health programs. Other related health programs may include a hospital, nursing facility or hospice.

4. Excellent oral and written communication and presentation skills.

5. Professional demeanor and appearance.

6. Proficient skills to promote excellent client relations and customer skills.

7. People management skills and the ability to network and manage a team.

8. Excellent organizational and time management skills.

Responsibilities/essential job functions: Employee must be able to perform the following essential job functions - with or without reasonable accommodations.

1. Determines and documents through evaluation if the client's needs do not exceed the level of in-home aide, companion, sitter, and/or respite services care.

2. If the client's needs exceed the in-home aide, companion, sitter, and/or respite services care, the agency shall make a referral to an appropriate alternative service.

3. Conduct initial assessments.

4. Develop the Plan of Care.

5. Verification of competencies of In-Home Caregivers.

6. Available during hours client care is being provided.

7. Demonstrate efficient teamwork with the staff.

8. Coordinate care for the client. *

9. Understand and perform the admission process plan of care. *

10. Coordinates services and schedules client visits.

11. Supervise field staff as indicated. *

12. Provide in-service education per Agency Director direction.

13. Attend seminars, conferences, workshops, and self-studies in areas that will enhance position (e.g. leadership, communication, geriatric care).

14. Perform the initial care visit and re-evaluate client needs and progress when needed. *

15. Coordinate and monitor all client care and services. *

16. Complete paperwork timely and efficiently. *

17. Maintain client records according to policy and procedure. *

18. Follow infection control policy in and out of the office. *

19. Provide quality of care to clients in their homes according to policy and procedures. *

20. Participate in the on-call schedule. *

21. Participate in and support quality improvement programs.

22. Supervise or designate a qualified individual to supervise the companion and sitter staff by contacting the client receiving care every three months and by making a supervisory visit to each client's place of residence at least every six months, with or without the companion and sitter's presence, and at least annually, while the companion or sitter is in the home providing services to the client.

23. The supervisory visit shall include a review of the client's general condition, monitoring progress and response to the services provided by the in-home aide, companion or sitter, and updates to the plan of care as needed.

24. Documentation of supervisory visits shall be maintained in the agency's records and shall contain the following:

· date of visit;

· findings of visit; and

· signature of person performing the visit.

Job Type: Part-time

Pay: $25.00 - $30.00 per hour
