About the job: The Admissions Nurse serves as the primary facility representative facilitating the admissions process and procedure for residents and their families, facility staff and with referral sources so as to provide for ease and efficiency in the transfer of the residents to the facility. In addition, this individual will serve as primary liaison between the facility and the community at large. Will be responsible for coordinating admission inquiries from referral sources and the public. Serving as the initial facility representative in inquiring parties for admission. Maintains relationships with residents and responsible parties and referral sources. Partners with the facility staff to ensure all objectives are met.

Essentials:

· Coordinates Resident Admissions:

· Screens resident’s healthcare benefits and obtains pre-authorization:

· Manages room occupancy and waiting list at the facility:

· Completes assessments of residents as necessary for Care Planning, MDS, CAAs, as directed.

· Participates in Interdisciplinary Care Conferences and rounds as directed.

· Coordinates and participates in internal and external marketing activities.

· Serve on committees, as assigned.

· Advises and recommends solutions to the department director of major problems or concerns that residents or their family members are experiencing.

· Maintains admission/discharge/referral/inquiry statistics/CRM.

Required Qualifications:

• Licensed Practical Nurse or Registered Nurse in the state of Florida required.
• Minimum one year of Case Management/LPN/RN experience, preferably in a long-term healthcare setting.
• Able and willing to work flexible hours, such as during evenings and weekends.
• Effective communicator with staff, families and outside agencies.
• Requires complex reasoning ability and negotiation skills
• Able to meet the requirements identified in the Essential Functions.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
• Ability to pass drug, criminal and employment screenings.

Preferred Skills:

• Bachelor’s degree in a related degree preferred.
• Has knowledge of long-term health care and public health settings.
• Familiar with Federal and State regulations and requirements for long-term care.
