The Prince George's County Department of Family Services is seeking a qualified applicant to fill a full-time Permanent Classified/Merit Community Development Assistant III position, grade A-17, within the Aging and Disabilities Services Division.
This is senior level para-professional position whereby the incumbent serves as an Homebound Assessment Worker, responsible for conducting face to face client assessments, satisfaction surveys, obtaining annual registration information, maintaining the client waitlist and entering such client data into database for reporting purposes, attend meetings regarding meal services for clients and addressing meal issues and resolving concerns, creating quarterly nutrition educational materials for distribution to home delivered meal participants..
The homebound assessment worker will perform the following duties:
Initiates client referrals and conducts priority screening for new and existing client.- Creates client files and maintain accurate records for all clients.
- Conducts home visits to ensure program eligibility of services.
- Assesses clients annually (home visits/face-to-face) and enter data into database for reporting purposes.
- Maintain client waitlist and communicate with food vendors to start homebound meal delivery services for approved clients.
- Communicate regularly with program receptionist, nutrition site staff and program manager actively expedite referrals for services.
- Answer phone calls regarding client referrals, and delivery concerns, cancellations and client holds.
- Send and respond to email correspondence to food vendors to expedite cancellations and delivery issues/concerns.
- Create and mail client start letter to new home delivered meal clients.
- Assist Data Entry Clerk with entering annual client assessment data into the database.
- Provide back-up phone and office coverage to Nutrtion Receptionist.
- Assist the Program Manager with special events and departmental activities and annual initiatives.
- Create mail merge letter to waitlist clients to provide resource information.
- Follow up with food vendors and Program Manager to expedite emergency home delivered meal services.
- Create and maintain donation log for home delivered meal donations from client participants.
- Attend Nutrition Virtual Site Meetings, Division Meetings and Partnership Meetings when required or requested.
- Assist with revision of home delivered meal policies and procedures set forth in the Older Americans Act, State and Federal Guidelines.
- Prepare accurate and concise reports, Program updates, training materials as requested.
- Willingly performs other tasks and duties assigned, which may not e specifically listed in this description; however, are within the general occupational category and responsibility level typically associated with the employee’s class of work.
What You'll Need:
Graduation from high school, supplemented by college level work in business, psychology or sociology.- One (1) year of progressively responsible experience in the nursing and assisted living field of long-term care.

PREFERRED QUALIFICATIONS:
- Experience in the field of Social Work, Mental Health, Human Services, Psychology, and other closely related fields
- Experience with conducting face-to-face home visits preferably experience assessing client needs and determining program eligibility and identifying resources for home delivered meal clients.
- Experience handling client date (files and data entry). The ability to demonstrate effective oral and written communications in the form of correspondence and report preparation.
CONDITIONS OF EMPLOYMENT: Upon selection, the candidate must:
- Meet all training and performance standards and demonstrate proficiency as required by the agency.
- Wear and use agency protective apparel and equipment in the performance of their assigned duties.
- Participate as an "Essential" employee in the department's snow and ice control program and other emergency or special activities as assigned.
DURATION OF ELIGIBILITY: Candidates will be selected from a temporary register of eligible candidates, which will become effective approximately four (4) weeks after the closing date. Once a selection has been made, the register will expire.
