Employment Supports Coordinator

Location #1 - Fridley, Coon Rapids, Brooklyn Center, Shakopee, Bloomington, E St Paul

Location #2 - St Paul, W St Paul, Mpls, Cottage Grove

  • The Employment Supports Coordinator (ESC) assists with ensuring that all assigned consumers have the means to develop, improve, and maintain desired employment opportunities. Maintains consumer records and ensures compliance with all data privacy regulations. Issues reports and maintains other records as required. Provides weekly updates to the consumer's interdisciplinary team members regarding the consumer's progress.
  • The Community Support Specialist (CSS) coordinates services with the employer and consumer to enhance work performance and job satisfaction through weekly visits to the worksite or offsite. Provides weekly updates to the consumer's interdisciplinary team members regarding the consumer's progress

Qualifications

For the ESC position, you must be at least 18 years old, and have a minimum of an Associates Degree (an emphasis in Social Services is preferred, but not required), knowledge of 245D documentation is preferred but not required, previous 2-year experience working with individuals that have developmental disabilities is preferred, have a valid driver's license with a reliable vehicle and proof of up to date auto insurance.

For the CSS position, you must be at least 18 years old, have a high school diploma/GED, have a valid driver's license with a reliable vehicle and proof of up to date auto insurance, and previous experience working with individuals that have developmental disabilities is preferred but not required.

Job Type: Full-time

Pay: $18.50 - $23.27 per hour
