DESCRIPTION:

In collaboration with the Congregational Leadership Team (CLT), serves sisters who reside outside the individual centers. Responsibilities include case management, engaging sisters in optimizing their healthcare and records management. Promotes a holistic approach to a healthy lifestyle with the focus on personal responsibility and health maintenance. Interdependence and communication are encouraged among the sisters within their local community group, the HCC and the CLT HCC Liaison, especially during times of illness and with significant decisions regarding health and well-being.

ESSENTIAL FUNCTIONS:

· Responsible for overseeing and coordinating plans for the changing and emerging health care needs of the sisters.

· Consults with sisters regarding healthcare concerns. Assists sisters in developing a healthcare plan which may incorporate both alternative health care approaches and traditional medical treatment.

· Provides opportunities and information to increase sister awareness of and use of wellness philosophy and activities.

· Participates in forming, evaluating, formulating and monitoring treatment(s) and service plans as needed and as requested by sisters.

· Maintains records with current medical information.

· Assists sisters in analyzing and evaluating feedback from practitioners to ensure services

rendered are improving the quality of life for Sisters.

· Coordinates transportation for sisters to medical appointments and treatments as needed.

· Establishes strong working relationships with physicians regarding sisters’ care.

· Available to assist in reviewing healthcare statements and/or invoices prior to payment being made if a charge is in question.

SKILLS AND ABILITIES:

· Must possess a willingness to learn and ability to appreciate the CSJ mission and charism, including an understanding of religious life, faith beliefs and spiritual practices supportive to the life of women religious.

· Must be able to incorporate the CSJ mission and charism into the healthcare process.

· Willingness to understand and support the Generous Promises of the Congregation of St. Joseph.

· Must possess the ability to work collaboratively with various personality profiles.

· Ability to maintain clear, effective, open and honest communication with sisters, Coordinators of Community Life, Administrators, Directors of Nursing and Director of Clinical Operations.

· Ability to multi – task and balance multiple projects within established timeframes.

· Detail oriented with ability to maintain strategic vision.

· Must possess strong assessment, professional writing and verbal communication skills.

EDUCATION AND EXPERIENCE:

· Licensed Registered Nurse or Practical Nurse.

· Understanding of the philosophies of Relationship Centered Living and of religious life.

· Demonstrates sensitivity to the aging process.

· Strong organizational and computer skills including proficiency with Microsoft Office.

· Valid Driver’s license required.

PHYSICAL ACTIVITY REQUIREMENTS:

· Primarily local travel at 50% with episodic needs for further travel.

· Some evening and/or weekend work

· Constant lifting up to 10 pounds

· Some lifting up to 25 pounds

Job Types: Full-time, Part-time

Pay: $27.85 - $31.00 per hour
