Nurse Practitioner - Wound Care

POSITION SUMMARY: Provides and manages continuous, medical care service to patients under direct consultation with the Medical Director of Hyperbaric Medicine and Wound Healing Center, and in collaboration with the private medical attending as indicated. Responds to the requests of the nursing staff as required to ensure patient care and safety.
POSITION REQUIREMENTS AND QUALIFICATIONS:
Education: Degree from an accredited Nurse Practitioner program.
Licensure/Certification: NYS Certified RN - NP (Adult or Family Nurse Practitioner). Trained in adult medicine. ACLS & BLS Certified, Wound Care Certified.
Skills, Knowledge or Abilities: Experience in patient case management required; hospital experience preferred. NP must be certified in wound care and maintain current collaborative practice agreement with the Medical Director of the center.
Physical Requirements: Work involves moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and loud noises. Moderate physical activity; requires handling of average-weight objects up to 20 pounds or standing/walking for more than four (4) hours per day. Work environment involves exposure to potentially dangerous materials and situations that require following extensive precautions and may include the use of protective equipment.
Clinical and Communication
- Consults and collaborates with the Hospitalist, private medical attending, and Hyperbaric Medicine and Wound Healing Center to provide continuous care and education to patients in the Emergency Department, inpatient and short stay service
- Performs medical, examinations, evaluations, assessments, diagnoses, and treatment to outpatients in the Diabetic and Wound Care Center.
- Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refers patients to specialists and to relevant patient care components as appropriate. Consults with Medical Director and / or attending whenever patient’s condition warrants.
- Coordinates the patient care activities of nursing and support staff as required.
- Completes discharge instructions to patients seen in the center.
- Participates in specified health promotion, education and prevention programs.
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and infection control standards.
- Makes best efforts to communicate within the group, other members, care team, patients and families.
- Promotes high quality, cost effective care based on the best medical evidence.
- Make best efforts to keep current with developments in medical management patients with diabetes and wounds and related areas through CME Programs, medical literature review and Medical Staff conferences.
- Performs miscellaneous job-related duties as assigned.
Administrative
- Maintains records and statistics, assessing, planning, implementing and evaluating wound, ostomy, and continence, and specialty tube care
- Knowledge of legal and ethical standards for the delivery of medical care.
- Knowledge of relevant drugs and non-pharmaceutical patient care aids. Ability to prescribe dosages and instruct patients in correct usage.
- Ability to deliver effective medical care within an outpatient environment.
- Maintain quality, safety, and infection control standards.
- Knowledge of related accreditation and certification requirements.
- Knowledge of current principles, methods, and procedures for the delivery of medical evaluation, diagnosis and treatment.
- Effective verbal and written communication skills.
- Ability to work both independently and in a team environment.
- Submit timely and accurate billing information as appropriate.
- Keep current with Medical Records documentation and respond in timely manner to requests for chart review and completion.
- Ability to develop and present educational programs and workshops.
