A Skilled Nursing Facility is seeking a special candidate that can do Social Services for a facility in Russellville Alabama. This position is responsible for coordinating services of new and returning residents/patients from home, hospital or other health care settings. This role in a 49 bed SNF will also require that you be able to assist with the discharge planning of patients as well as organizing and conducting care plan meetings.
Duties and Responsibilities:
- Maintain and be the line of communication with hospital ,discharge planners, families and facility staff to coordinate new and re-admissions
- Manage the inquiry process professionally, timely and with appropriate follow-up
- Conduct tours of the facility to prospective residents, family members and marketing clients
- Provide excellent customer service, while fielding inquiries regarding our services and or issues with existing patients/families including room management
- Maintains along with the marketing director budgeted census goals
- Support the facility’s philosophy of care and strive to achieve its goals and objectives
- Provide discharge services and support for residents discharging
- Attend and organize care plan meetings
Job Requirements:
- Must be able to work in a fast paced environment, juggling many different tasks with a sense of urgency
- 1 year experience minimum Admissions and/or Social Services experience in senior care
- Must be able to project a warm, friendly and sympathetic interest to all
- Must possess the ability to deal tactfully with personnel, residents, family members and visitors.
- Must possess the ability to make independent decisions when circumstances warrant such action ; Must be able to be flexible to meet changing situations
