Compassionate Home Health Care is looking for a motivated and enthusiastic RN like you, to join our experienced team as a Home Health Care RN Case Manager in the Denver Metro area.

We provide Home Health Care in many Assisted Living Communities, helping to limit drive time required to complete visits. All of our management staff are also clinicians and so the support and orientation we offer, is exceptional.
We provide flexible schedules, competitive pay, paid orientation/training, paid time off, and full benefits, to those who qualify. We pay our staff ‘hourly’, and not per-visit, so they are compensated for their driving time, documentation time, care coordination, etc. Our productivity expectation is lower than other home health care agencies, which allows our RNs to be paid more hours per week. Further, we consider the additional time and skill needed for Assessments and Evaluations and we increase our compensation accordingly.

Job Duties
· Flexible schedule to assist with new starts of care with schedule Saturday/ Sunday and to include Friday/ Monday depending on hours desired
· Performs and oversees admissions (SOC), resumptions, re-certifications, and discharges by evaluating the patient in their home or community
· Formulates/oversees plans of care (POC)/Reviews all plans of care for accuracy, and appropriateness of documentation
· Performs/oversees skilled nursing care in the patient’s home as necessary & provides teaching to all team members on skills and nursing care including but not limited to documentation
· Teaches and explains the health treatment and teaches the patient and family how to provide care at home
· Evaluates patient’s needs and the effectiveness of care and makes changes and adjustments to POC as necessary
· Coordinates schedules with team members
· Communicates and problem solves with the interdisciplinary Team members to ensure the patient receives the highest quality of care
We are looking for RNs who possess the following:
· Commitment to providing quality care while collaborating with our community organizations in patient care.
· Ability to work well within a team environment to collaborate daily with team members, Physical Therapists, Occupational Therapists, Speech-Language Pathologists, MSW’s and the management team.
· Eagerness to gain higher level of opportunities such as certifications and further education.
· Provision of On-Call RN services per agency policy
· Attendance at all staff meetings as able
· Close coordination of care with patients on a 1:1 level with strong support from our patient care team.
· Ability to work independently and on your own, and to request guidance when needed
· At least one year of recent Home Health experience is preferred but not mandatory.
· Ideal candidates must have excellent interpersonal skills and a passion for home health.
· Patience, motivation and a positive attitude
· Physically able and willing to lift patients for patient care when needed
· Ability to resolve problems, set priorities, and function independently.
· Previous hospital, hospice, home health or SNF experience is a plus.
· Visit numbers may increase with Willingness to drive within a larger geographic area.
Requirements
· Associates Degree in Nursing (Required)
· Reliable transportation, Valid Driver’s License & Car Insurance
· Current CPR Certification
· Current Colorado Registered Nurse License
Locations: Currently recruiting in North Denver Metro including Littleton

Pay: $36 - $42 per hour, based on experience
